To complete the application, you’ll need to download and fill out this form: E-595E Streamlined Sales and Use Tax Agreement Certificate of Exemption. As you fill out the form, please remember to:
1. Add your business name for Name of Purchaser.
2. Add your business address.
3. Add your business tax ID number or FEIN (Federal Employer Identification Number).
4. Check the appropriate option under Type of Business. For example, most of our partners use 10. Retail trade.
5. Check the appropriate option under Reason for Exemption. For example, most of our partners use G. Resale (include your state tax identification number if applicable).
6. Use our address for Seller’s address: Earthy Select, 825 C Merrimon Ave. #213, Asheville, NC 28804.
7. Sign the form.
